The dictionary definition of "what" is "asking for information specifying something". Therefore, "what's up" could literally mean that the asker is asking for information specifying the object, place, organism, ect. that is towards a higher place or position. Now, to answer your original question: I could say that not much is going on in my life but I'm doing pretty well. Then, i could use this deep and meaningful phrase in return: "how about you, what's up in your life?". OR I could take this literally. I could look up, and spy a wooden ceiling that is above me. Then, I would say that a wooden ceiling is up. However, one cannot be sure whether you are asking for what is directly above me or what is even higher. For then I could say that above the wooden ceiling is my roof, and above that is the sky, and above that is space, and above that is goodness knows what.

Perhaps this phrase should be slightly altered. This could confuse many of the world's population, hurting their brains and possibly causing mass brain explosions or collapses. This would be a rather unfortunate ordeal, and it would be best to minimise brain damage. Perhaps, askers could specify WHICH "what's up?" They are referring to when asking the simple-seeming phrase. This would make life easier and, as well as minimise brain damage, save embarrassment, trauma and also time. This could embarrass people because they may take the phrase the wrong way and cause themselves embarrassment, trauma and life scars. It could also save time. Every second of life counts, and time wasted getting angry, sad, confused, ect, over a two-word phrase is unnecessary. You would not want to look back in 80 years time, thinking, "oh how I wasted those minutes/hours/days/weeks/months/years/decades arguing over the phrase "what's up?" In conclusion, I would best stick to any other phrase such as "how are you?", "anything new?", etc. , just not the traumatic, oh-so-terrible, life-threatening phrase, "what's up?"
